A Verified card means the provider has a clinician license confirmed for the state you searched, a state pharmacy license on file, an NABP record that resolves, a facility kind we can identify (503A, 503B, or none), and itemized pricing we have captured. The four checks plus the pricing line are the same fields shown on /results — Verified is the visible signal for that combined state, not a separate score.
